Community Health Professionals (CHP) is a comprehensive, nonprofit home health and hospice organization serving Northwest and West Central Ohio since 1975. CHP delivers skilled home health services, hospice, visiting nurses, inpatient hospice, private duty, adult day care, physical therapy, occupational therapy, speech therapy, and related services. We have approximately 131 full-time and 306 part-time and temporary employees. CHP services are nationally accredited for quality by CHAP and rank in the top 20 percent nationally for patient satisfaction.

Positive Attitude

Valid driver license, reliable transportation and proof of auto insurance
RNs: Current Ohio RN license and CPR certification
Good communication skills. Must be able to lift 50 pounds, assist with 100 pounds
