Intake Processor

CHP Home Care & Hospice, a nonprofit organization, is seeking a detail-oriented individual to ensure timely, accurate, and compliant coordination of referrals, payer verification, and physician documentation for home health and hospice services.

The Intake Processor serves as the central communication point between referral sources, physicians, patients, families, and agency clinical leadership to facilitate acceptance, scheduling, and admission readiness.

Our ideal candidate is an experienced LPN, social worker, or medical assistant with strong verbal communication, time management, customer service, and organizational skills.

Job Responsibilities

• Review referral documentation for completeness, diagnosis, payer source, physician information, and requested services
• Confirm referral appropriateness, staffing availability, geographic coverage, and payor acceptance
• Coordinate collection of required physician orders, certifications, documentation, and hospice CTI
• Communicate with patients, families, hospitals, physicians, facilities, and referral partners
• Responsible for navigating patient medical records via hospital portals
